Lourdes Opens Two New Chronic Care Centers

Centers in Willingboro and Marlton Aim to Help Patients Manage Chronic Obstructive Pulmonary Disease

WILLINGBORO, NJ—Lourdes Medical Associates recently opened two new Chronic Care Centers, located at its Willingboro campus and in Marlton, NJ. The centers were established to bridge the gap between the hospital and doctor’s office, with a goal of helping patients who have chronic obstructive pulmonary disease (COPD) stay out of the emergency room and reduce unnecessary hospital admissions.

The centers currently focus on managing patients with COPD, however, services to treat other conditions may be added in the future.

“The Chronic Care Centers focus on helping COPD patients manage their condition and improve their quality of life. That first starts with keeping patients out of the hospital,” said Candace Bromley, PA-C, lead for the Chronic Care Centers at Lourdes. “When patients have serious medical conditions, or are in transition between health facilities, they often need intense, multidisciplinary treatment and disease management. These cases require more attention and resources. Our hope to transform treatment for these complex patients by providing everything they need in one coordinated setting.”

Patients will have improved access to care with center visits available either the next day or the same day. In addition to on-site care management, the centers integrate social work needs, health education and coaching, and medication management within the visit to support patient health. Telehealth monitoring will allow clinicians to have a virtual visit with patients to regularly assess their vital signs and well-being. The centers work closely with clinicians in pulmonary rehabilitation, physical therapy and occupational therapy to care for patients.

Lourdes pulmonologist Thomas Nugent, MD, FCCP, will also provide care at the centers. “Patients who have COPD, who may or may not also have diabetes, chronic kidney disease, dementia, or exacerbation of heart failure, are welcomed to come in for an assessment,” said Dr. Nugent. “Our goal is to stabilize the patients’ overall condition, improve their quality of life, and teach patients self-management skills they can apply in their home environment.”

COPD is a chronic lung disease that includes emphysema and/or chronic bronchitis. COPD usually progresses gradually, causing limited airflow in and out of the lungs. COPD also adds demand to the heart.

The Chronic Care Centers work to provide education, treatment and the follow-up care needed to help prevent flare ups and hospitalizations. Services offered include:

  • Extension of care following hospital discharge
  • Intensive hospital follow-up and education
  • Spirometry testing
  • Nebulizer treatments
  • Vaccines/immunization
