Project H.O.P.E.
Project H.O.P.E. (Homeless Outreach Program Enrichment) began as a volunteer
outreach initiative designed to meet the primary needs of the unsheltered
homeless population of Camden City. Client Services include:
- Comprehensive medical care
- Physical Exams
- Referral and Specialty Care
- TB Testing
- HIV Testing
- STD evaluation and treatment
- Drug and Alcohol Counseling
- Employment Counseling
- Financial Counseling
- Food and clothing distribution and referral
- Housing referral services
- Social Service
- Street Outreach
Bergen Lanning Health Center
Established in 1999, the Bergen Lanning Health Center serves as a primary medical home for the homeless. It is open Monday through Friday, from 8:30 a.m. to 4:30 p.m.. The location is 439 Clinton Street, Camden, NJ.Satellite Clinics
Project H.O.P.E. operates a weekly satellite clinic at Volunteers of America Aletha Wright's Transitional Living Program for the shelter's residents.Services of the satellite clinic include: physicals, social service case management, non-emergency medical care, and referral services.
Mobile Clinic Unit
The Mobile Clinic Unit visits sites throughout Camden City to provide primary medical care, social services, assessments as well as on-foot outreach to serve the hard-to-reach population.Film Project
In January 2006, Project H.O.P.E. was awarded a grant to develop educational videos for homeless. In March 2006, a film student from the University of Utrecht in The Netherlands joined Project H.O.P.E. to develop a series of educational health films. Members of the homeless population are the actors and provided their input in developing and writing the scripts. The idea behind this decision is that the films will become more familiar to the audience, creating a high level of character identification. This then will make the message more credible, thus enhancing the learning effect of the audience. The topics of the education films include Diabetes, Asthma triggers, Nutrition, and Housing Assistance.Asthma Collaborative
In October 2005, Project H.O.P.E. began participating in the Asthma Collaborative, which is part of the Health Resource Service Administration (HRSA) Health Disparities Collaborative. The Health Disparities Collaborative is a national effort to achieve system change in the delivery of primary health care. The Asthma Collaborative is chronic care model that will help to eliminate asthma disparities within our patient population by using evidence-based clinical guidelines.Supporting Project H.O.P.E.
