The LIFE at Lourdes Service Locations
The purpose of the LIFE program is to allow elders to "age in place," to live in their own homes for as long as possible. The LIFE model honors what the frail elderly want: to stay in familiar surroundings, maintain autonomy and maintain a maximum level of physical, social and cognitive function. LIFE at Lourdes provides services in:
- The Participant's Home
- The LIFE Center
- A Rehabilitation Facility
- Long-Term Care Facility
