CAMDEN, N.J. -- Lourdes Health System in conjunction with NJFamilyCare, will host enrollment sessions for qualified children and families in recognition of Cover the Uninsured Week. Sessions will be held from 10 a.m. to 3 p.m. on Monday, March 23 at Our Lady of Lourdes Medical Center, 1600 Haddon Avenue, Camden; and from 10 a.m. to 3 p.m., Thursday, March 26 at Lourdes Medical Center of Burlington County, 218 Sunset Road, Willingboro. Representatives from AmeriChoice, one of several NJFamilyCare providers, will be available to help eligible applicants review enrollment requirements and apply for appropriate coverage.
Cover the Uninsured Week is a national campaign sponsored by the Robert Wood Johnson Foundation to create awareness about the plight of the uninsured and to provide resources to families and individuals in need. NJFamilyCare, which provides free or low-cost coverage for uninsured minors and for low-income parents of eligible children, is accepting new applications from parents at or below the federal poverty level. Eligibility is based on a family's size, including children and parents, and monthly income. Immigrants whose documentation grants them permanent residency in the United States are eligible.
Documentation needed by applicants to complete their enrollment includes:
- personal identification of parents and children, such as Social Security cards and birth certificates;
- proof of income, including the applicant's last four pay stubs;
- proof of residency, such as utility or phone bills or a notarized letter from a landlord.
